Notes for Applicants

  1. The University's graduate certificate is a unique document. No duplicates are issued. However, graduates may request for a replacement certificate in case of loss or damage to the original certificate. Replacements due to name changes after graduation are not permitted.
     
  2. Graduates are allowed to possess only ONE valid certificate copy, including any replacement certificate, for each degree and/or diploma at any given time. The original copy of certificate will be deemed invalid once the replacement copy is issued.  If a previously reported lost certificate is subsequently found, the graduate must return the invalid original copy to the Graduate School Office.
     
  3. A replacement certificate shall, in general, follow the current format of the graduate certificate1, with an additional statement “This certificate is reissued on (DD/MM/YYYY) [in Chinese:本證書於YYYY年MM月DD日補發]”. The replacement shall bear the signatures of the current University officers.
     
  4. Replacement certificates are issued on three specific dates in a calendar year as tabulated in the 2nd column of the 2025-26 schedule as follows:

  1. To apply for a replacement, graduates must submit an online application and upload required supporting documents to the designated system. The Alumni Affairs Office provides a quick reference guide for MyCUHK accounts and alumni email services.
  1. Required supporting documents for replacement applications:

    (a) Copy of the HKID card^ / passport*;

    (b) If original certificate is damaged, return it to the Graduate School Office;

    (c) If original certificate is lost, provide original copy of relevant documentary evidence (e.g. police report or notarised statement).  

    ^ HKID number should match with the University's registration records.
    * Graduates who have lost or do not have a HKID card should provide a copy of their passport. The name and date of birth printed thereon should be the same as the registration record at the University.

 

  1. The replacement fee is HK$800 per certificate, payable by credit card through the application system. This fee is non-refundable and non-transferrable once paid.

  1. If only the degree / diploma name (but not the programme name) was shown on the original certificate, the replacement certificate, if issued, shall also bear the degree / diploma name only.
